為配合內政部移民署實施外來人口新式統一證號換發作業，如果您已經換發新式統一證號者，請您本人親持新式居留證、存款帳戶原留印鑑、舊金融卡至原開戶分行臨櫃辦理變更帳戶資料，以維護您的權益。

如果您未申請網路銀行、金融卡，您可以下載下列表單，填妥資料並加蓋存款帳戶原留印鑑及檢附新式居留證影印本，以郵寄方式寄至原開戶分行辦理變更資料。

您完成換發新式統一證號後，建議您同步更新相關資訊予撥薪雇主、證券公司、公共事業費機關、稅捐機關…等單位，以利該單位更新資訊並完成入扣帳作業。

提醒您，本年度所得稅結算申報時，請您注意申報之統一證號與金融機構留存之證號是否相符，以維護您的權益。

In order to apply to the competent authority for the replacement operation of Ministry of the Interior National Immigration Agency Republic of China (Taiwan). If you have already been issued a new UI No., you shall bring your R.O.C.(Taiwan) Resident Certificate with new UI No., the original seal, ATM card to the original branch of the account to update your UI No. information.
If you haven’t applied for internet banking or ATM card, you can download the above application form, fill in the information, affix the original seal, and attach a photocopy of your R.O.C.(Taiwan) Resident Certificate with new UI No., and send it to the original branch of the account to handle the change of information.
After replacing your UI No., we suggest you update your newUI No. to payroll employers, securities companies, public
utility fee agencies, taxation agencies and other agencies, so as to facilitate the subsequent process of automatic debit or direct fund transfer.
This year’s income tax settlement and declaration period will expire soon. For your rights, please confirm that your declared UI No. matches the certificate number retained by the financial institution.
